Start with the actual studio agreement
Collect the agreement before asking for a certificate. Identify the legal entity hiring you, each teaching address and the activities you will deliver. A studio may request an additional-insured endorsement; the certificate and the endorsement serve different purposes. Ask the provider to explain exactly what has been added.
Does the requested endorsement apply to my instruction at this studio?
Follow the instructor, not just the room
Describe reformer work, mat classes, home visits, online lessons and substitute teaching separately. A studio’s own policy does not establish what protection an independent instructor has. If you teach for several studios, list them and ask how newly added locations are handled.
What must I report before teaching at another venue?
Make the next conversation useful
Bring the current documents and explain any changes. These details are a useful starting point:
- Teaching qualifications and modalities
- Studio addresses and rented-space agreements
- Reformer and equipment replacement values
Ask the provider to connect its answer to the actual policy or eligibility terms, including exclusions, dates and reporting conditions. Keep the confirmed answer with the offer.
